WesleyLife Home Health provides comprehensive home health care services throughout central Iowa, serving clients in Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair, and Poweshiek counties. Responsibilities

As a Visit Nurse (RN), you will play a key role in supporting the health, safety, and independence of the clients we serve. Your responsibilities will include:

Clinical Visits & Assessments

• Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in‑home visits.
• Evaluate client progress, response to treatment, and ongoing needs to ensure high‑quality, person‑centered care.
  
  

Education & Family Support

• Counsel clients and their families on health conditions, care plans, medication management, and other related needs.
• Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
  
  

Communication & Coordination

• Maintain ongoing communication with the Nurse Case Manager regarding:
• Updates to healthcare provider orders
• Changes in client condition
• Incident reports
• Necessary medical equipment or supplies
• Collaborate with the interdisciplinary team to ensure seamless, coordinated care.

• Competitive Pay: The pay rate for a fulltime position is $65,375 - $88,240 / year. The starting pay rate will be based on which position you’re applying for and your years of experience.

• Daily Pay: Need your wages before payday? Use our free Daily Pay app to access your earnings when you need them.
• Mileage Reimbursement at 58.5 cents per mile
